Pflegeversicherung – the mandatory state insurance for cases where someone needs long-term care (Pflegefall). It helps cover the costs of medical and domestic care for people who can no longer fully look after themselves due to age, illness or disability. This insurance is part of the social security system in Germany and is compulsory for everyone insured with a Krankenkasse (statutory health insurer) or a private health insurer.
What does Pflegeversicherung cover?
Pflegeversicherung provides financial support for various forms of care:
- Home care – nursing services, help around the house, home adaptation.
- Partial residential care – for example, day centres for older people.
- Full residential care – placement in a care home or specialist facility.
- Social benefits – if care for a person is provided by their relatives.
The amount of aid depends on levels of care need (Pflegegrad), of which there are 5 in Germany: from light assistance (Pflegegrad 1) to complete dependence on outside care (Pflegegrad 5).
How much does Pflegeversicherung cost?
Contributions are calculated as a percentage of gross salary and depend on the number of children. Rates 2026:
- 4,2 % — for those without children. This is the base rate of 3.6 % plus a supplement of 0.6 percentage points. The supplement is payable from the month following the person’s 23rd birthday; those born before 1 January 1940 do not pay it.
- 3,6 % — basic rate for parents: with one child, and also when all children have reached the age of 25.
- From the second to the fifth child the rate falls by 0.25 percentage points for each — until the end of the month in which the child turns 25. For two children: 3.35 %; for three: 3.10 %; for four: 2.85 %; for five or more — 2,60 %.
- The employer pays half the basic rate (1.8 %). The employee pays the childless supplement themselves. In Saxony, the employee’s contribution is 0.5 percentage points higher.
- Contributions are calculated up to the ceiling of €5,812.50 per month (€69,750 per year). The maximum contribution is €209.26 per month, or €244.13 for those without children.
Pflegegrad in Germany: levels of care dependency
Pflegegrad is a system for assessing a person’s care needs, on the basis of which the amount of insurance payouts under long-term care insurance (Pflegeversicherung) is determined. In Germany there are 5 levels of care (Pflegegrade), which depend on the level of loss of independence.
The level of care needs is assessed by the medical service MD (Medizinischer Dienst) for those insured in the public system, or Medicproof for private insurance policies. Specialists carry out an assessment and evaluate the extent to which a person can manage everyday tasks independently. The main criteria are:
- Mobility (can the person move around without outside help).
- Cognitive and communicative abilities (presence of dementia, disorientation).
- Ability to look after oneself (can dress, eat, and care for oneself).
- Daily life management and social contacts.
5 Pflegegrad levels
Home care payments, 2026 (euros per month). The amounts have applied since 1 January 2025 and did not change in 2026: the next indexation is set by law for 1 January 2028.
| Pflegegrad | Condition | Financial allowance care by relatives, Pflegegeld (care allowance) | Care services Pflegesachleistung (professional care services) |
|---|---|---|---|
| 1 | Minor restrictions | not paid | not paid |
| 2 | Moderate care needs | 347 € | 796 € |
| 3 | Significant restrictions | 599 € | 1.497 € |
| 4 | High care needs | 800 € | 1.859 € |
| 5 | Greatest dependency on care | 990 € | 2.299 € |
Important clarification regarding Pflegegrad 1. The phrasing “nothing is paid” is inaccurate: a cash allowance and payment for care services are indeed not due, but the Entlastungsbetrag (relief amount) of 131 euros per month is payable at every Pflegegrad, including the first. In addition, care home residents with Pflegegrad 1 receive a subsidy of €131 per month.
Cash benefits and care services can combine: if half of the service allowance has been used, half of the financial benefit is paid.
All benefit amounts for 2026 are listed in the Federal Ministry of Health (BMG) overview.
The higher the Pflegegrad, the more financial support can be received for organising care. If relatives care for a person themselves, allowances are also paid to them.
It is important to understand that Pflegeversicherung only covers basic costs, and in most cases a person or their relatives have to pay extra for care out of personal funds. To cover the difference, you can take out a private Pflegezusatzversicherung (supplementary long-term care insurance), which will provide a higher level of protection.
Pflegeversicherung is an important element of social protection in Germany, but additional financial planning is often required to fully cover all costs.
Private nursing supplementary insurance (Pflegezusatzversicherung) in Germany
Pflegezusatzversicherung is private supplementary insurance for cases where you need long-term care. It is designed to cover costs that are not reimbursed by the statutory Pflegeversicherung.
What is a Pflegezusatzversicherung for?
State long-term care insurance (Pflegeversicherung) only covers part of the care costs, but in reality, the expenses for carers, care homes and medical care are significantly higher. The shortfall becomes a financial burden for the patient themselves or their relatives. Supplemental long-term care insurance (Pflegezusatzversicherung) helps avoid these expenses and ensures quality care without unnecessary out-of-pocket spending from personal savings.

Is it worth taking out a Pflegezusatzversicherung?
Supplementary long-term care insurance is particularly useful if you do not want the costs to fall on your family’s shoulders or be forced to use your personal savings. This insurance ensures financial independence in old age and guarantees quality care without compromise.
What else does the insurance cover besides the monthly payments
Apart from the financial allowance and care services, there are separate budgets that are often unknown and simply go unused.
| What | How much | Conditions |
|---|---|---|
| Relief amount Entlastungsbetrag | €131 a month | For care at home, at every Pflegegrad, including the first. Intended use: carer respite services. Unspent amounts roll over to the next half-year |
| Respite care and short-term care Verhinderungspflege and Kurzzeitpflege | €3,539 a year - a single, unified budget | Since 1 July 2025, the two previous budgets have been combined into a single annual budget (§ 42a SGB XI). It requires at least Pflegegrad 2. For each type — up to 8 weeks a year |
| Half of the allowance during respite care | 50 per cent of the financial allowance | It continues to be paid during periods of respite or short-term care, for up to 8 weeks for each type |
What is important here in practice. Prior approval for respite care is not required — you can pay for the service and submit documents for reimbursement. The deadline for submitting a reimbursement claim is the end of the following calendar year. If care is provided by relatives up to the second degree of kinship or cohabiting persons, the reimbursement cap is lower: a cash allowance for two months plus documented expenses.
Care home: how much the family really pays
The insurance payout for residential care (ranging from €805 to €2,096 per month depending on the Pflegegrad) only covers care. Accommodation, board and the facility’s investment costs must be paid by the individual themselves.
| Component | On average across Germany, euros per month |
|---|---|
| Care co-payment, the same for all Pflegegrade, after the 15 % supplement in the first year einrichtungseinheitlicher Eigenanteil, including staff training costs | 1.775 € |
| Accommodation and board | 1.068 € |
| Capital expenditure of the institution | 521 € |
| Total for the first year of stay | 3.364 € |
Averages according to the Verband der Ersatzkassen (association of substitute health insurers) as of 01/07/2026. The regional variation is significant: ranging from approximately €2,891 in Saxony-Anhalt to €3,761 in Bremen.
A supplement that reduces this amount over time
The longer a person lives in a care home, the greater the proportion of their own contribution towards care costs that is covered by the Pflegekasse (long-term care fund) under § 43c SGB XI:
| Length of stay | The Pflegekasse covers |
|---|---|
| up to 12 months | 15 per cent of the resident’s own share of care costs |
| more than 12 months | 30 per cent |
| more than 24 months | 50 per cent |
| more than 36 months | 75 per cent |
The supplement applies only to the resident’s own share of care costs. It does not reduce accommodation, board or investment costs. It applies from Pflegegrad 2.
New from 1 July 2026: The Pflegekasse calculates the supplement itself, based on the care home’s data, and transfers it directly to the care home. The resident is billed an already reduced amount – there is no need to submit a separate application.
Are children responsible for the costs of caring for their parents
This is one of the most common fears, and it is largely outdated. Since 2020, the rule has been: maintenance claims against children (and against parents) are not taken into account, if the annual aggregate income of the respective person does not exceed €100,000 — Section 94, subsection 1a of the Twelfth Book of the Social Code (SGB XII).
- The threshold is calculated on an individual basis, and not in total for all children.
- There is a presumption: It is assumed that the income does not exceed the threshold. The social welfare authority is entitled to request information only if there are sufficient grounds to believe otherwise.
- It is about the total annual income within the meaning of Section 16 of the Fourth Book of the Social Code (SGB IV), and not the gross salary under the employment contract.
In other words: at a standard level of income, children are not required to contribute towards the costs. Proceedings only commence once this threshold is exceeded.
How to obtain a Pflegegrad: procedure and timeframes
- Application to the Pflegekasse. Submit it to the Pflegekasse at your Krankenkasse, either verbally or in writing. The date of submission is important: payments are calculated from the month in which the application is submitted, not from the date of the medical examination.
- Examination. For those insured in the public system, it is carried out by the MD medical service, and for private patients, by Medicproof. The examination takes place at the place of residence. Refusal to let the expert in gives the Pflegekasse the right to refuse services.
- Assessment across six areas: mobility, cognitive and communication skills, behaviour and mental health, self-care, coping independently with the demands and challenges of treatment, organising daily life, and social interactions.
- The decision must be made no later than 25 working days from the date the application is received, in writing (§ 18c SGB XI). In the event of a delay, the Pflegekasse must pay the applicant 70 euros for each commenced week of delay if the delay is its fault.
- The expert report is sent together with the decision, if you do not mind it being forwarded. Always request it: without an expert report, it is almost impossible to challenge the decision.
- An objection must be submitted within one month of receiving the decision. Rejections and too low a Pflegegrad are common, and objections are frequently upheld.
A tip that noticeably increases the chances: One to two weeks before the assessment, keep a care diary detailing how much time is spent on each activity every day. An expert only sees a person once for an hour, and in the presence of strangers, people often demonstrate more independence than on a normal day.
Additional insurance: three types and state subsidy
| Type | How does it pay | Who it is suitable for |
|---|---|---|
| Daily allowance Pflegetagegeld | Fixed daily or monthly allowance, can be spent on anything, no receipts needed | Most people. The most flexible option, especially if relatives take on the care |
| Reimbursement of expenses Pflegekostenversicherung | Percentage of actual professional care service invoices | For those who count on professional care and are ready to collect the paperwork |
| Care annuity Pflegerentenversicherung | Lifetime annuity upon the onset of care needs, often with a savings component | For those for whom the savings element is important. Usually more expensive for the same level of cover |
Pflege-Bahr: state-subsidised care policy
This is a separate tariff with fixed conditions set by law (§§ 126, 127 SGB XI). The main thing you need to know is:
- State top-up: €5 a month with a personal contribution of at least €10 per month. The top-up does not count towards the minimum contribution.
- The insurer is obliged to accept anyone: no medical questions, no risk loadings and no pre-existing condition exclusions. This is a key benefit for people with chronic conditions who cannot get standard policies.
- Benefits are paid at all Pflegegrade, with at least €600 a month at Pflegegrad 5.
- The waiting period is up to 5 years. This is the main drawback: if the need for care arises in the first few years, there will be no payouts.
- The state top-up is not payable to persons under 18 years of age and those who at the time of concluding the contract are already recognised as being in need of care.
- Only one subsidised contract per person.
How to choose between Pflege-Bahr and a standard tariff: If you are healthy and under 50, a standard tariff usually provides better cover for the same money. If you have medical conditions that lead standard insurers to refuse cover or charge higher premiums, Pflege-Bahr is often the only available option.
Who is required to be insured
- State system: Everyone who is compulsorily insured with a statutory Krankenkasse (§ 20 SGB XI) is insured. Care insurance automatically follows health insurance.
- Private medical insurance: obligation to take out private long-term care insurance with the same insurer (§ 23 SGB XI). The benefits must be equivalent to those of the statutory scheme: instead of benefits in kind, a cash reimbursement of the same amount is paid. Another insurer can be chosen within six months.
- Children and non-working spouses are insured free of charge in the state system as part of family insurance. In the private system, children are insured free of charge and adults pay their own contribution.
