Ottonova is a digital insurer from Munich. The contract is concluded online and bills are submitted as a photo in the app: no paper correspondence, no sending of originals.
Zahn 100 — the top tariff of the range (First Class). It pays 100 % for Zahnbehandlung (dental treatment), Zahnersatz (dental prosthetics), implants and prophylaxis, has no Wartezeit (waiting period) and asks only three health questions. Below is the breakdown based on the official Bedingungen (terms and conditions) — Tarif- und Versicherungsbedingungen ottonova Zahn 100 (Gen3), Ausgabe 11/2023 — and on the Beitragstabelle (premium table) published on ottonova.de.

Who it suits and who it does not
Suitable for:
- anyone who wants maximum cover and does not want to pay for Zahnersatz out of pocket;
- anyone who does not want paper correspondence: application and bills go through the app;
- parents: children’s Kieferorthopädie (orthodontics) is paid from KIG 1 (the mildest statutory severity grade) — where the Kasse (statutory health insurer) pays nothing at all;
- anyone who is afraid of treatment: general anaesthesia, nitrous oxide, hypnosis and acupuncture are included in the tariff;
- anyone switching from another dental policy — the Summenbegrenzung (cap on total payouts) drops from four years to two.
Not suitable:
- anyone missing more than one tooth: from the second missing tooth Ottonova does not conclude a contract;
- anyone who needs adult Kieferorthopädie without an accident — it is not covered here;
- anyone who wants to fix the premium for life: it rises when you move into the next age band;
- anyone not in a German Krankenkasse (statutory health insurer): the tariff is open only to GKV members.
What Zahn 100 covers
| What exactly | Ottonova Zahn 100 |
|---|---|
| Zahnbehandlung: fillings, root canals, Parodontose (periodontitis), apicoectomy | 100 % |
| Zahnersatz: crowns, bridges, veneers, Inlays and Onlays | 100 % |
| Implants and Knochenaufbau (bone grafting) | 100 %, no limit on quantity |
| Professional cleaning (PZR), caries risk diagnostics, Fissurenversiegelung (fissure sealing) | 100 %, up to 90 € per session and up to 180 € per year |
| Functional diagnostics and therapy, splints (including DROS) | 100 % |
| Kieferorthopädie for children up to 18, KIG 1–5 | 100 %, up to 2 000 € over 10 Versicherungsjahre (policy years) |
| Orthodontics for adults | only if the Kasse pays or after an accident |
| Additional Schmerzausschaltung (pain control): general anaesthesia, nitrous oxide, hypnosis, acupuncture | 100 %, up to 300 € per year |
| Heil- und Kostenplan (treatment and cost plan) if the Kasse has not paid for it | 100 % |
| Treatment abroad | yes, within German fee rates |
| Wartezeit | no |
| GOZ (dental fee schedule) factor | up to the GOZ Höchstsatz (maximum factor) — 3,5 |
“100 %” here means: the insurer tops the bill up to the full amount after the Kasse has paid its share. A separate clause in the Bedingungen: if you choose a dentist without Kassenzulassung (no accreditation with the statutory scheme) and the Kasse therefore pays nothing, Ottonova reimburses 70 % of the bill rather than 100 %. For purely private practices that is an important detail.
GOZ factor: up to 3,5, anything above at your own cost
A German dentist bills according to the GOZ — Gebührenordnung für Zahnärzte (the dental fee schedule). The base price of a service is multiplied by a factor from 1.0 to 3.5 (§ 5 Abs. 1 GOZ), with 2.3 considered average. Anything above 3.5 is only possible with a separate written agreement with the patient — a Honorarvereinbarung under § 2 GOZ.
Ottonova states in its Bedingungen explicitly: costs are reimbursed “bis zu den Höchstsätzen der jeweils gültigen Gebührenordnung”, that is up to factor 3,5. That is the market standard — most tariffs work this way. But if your dentist bills at 4,0 or 5,0, you pay the difference yourself.
Ottonova’s three tariff classes: Economy, Business, First
The range has three levels. Zahn 100 is the top one. As at 01/2024; premiums are shown as an example for age 29.
| What is covered | Economy Zahn 70 | Business Zahn 85 | First Zahn 100 |
|---|---|---|---|
| Dental treatment | 100 % | 100 % | 100 % |
| Zahnersatz, implants, Inlays | 70–100 % | 85–100 % | 100 % |
| PZR per year | up to 70 € | up to 140 € | up to 180 € |
| Kieferorthopädie (orthodontics) for children | no | 85–100 % | 100 % |
| Orthodontics for adults | no | no | yes, conditional* |
| General anaesthesia, nitrous oxide, hypnosis | no | no | up to 300 € per year |
| Premium at age 29 | 8,80 € | 12,68 € | 20,18 € |
* Adult Kieferorthopädie is paid at 100 % in Zahn 100, but only if the Krankenkasse has accepted liability or if the treatment follows an accident that occurred after the contract began.
The difference between Economy and First is 11,38 € a month, about 137 € a year. A crown costs 600–1 200 €, and with Zahn 70 your Eigenanteil (co-payment) is 180 to 360 €. The premium difference pays for itself with the first piece of Zahnersatz.
Zahn 100 and Zahn 100+: the difference
Every level also comes in a plus version, and “plus” always means the same thing — a higher limit for professional cleaning. The extra 5 % for Zahnersatz comes not from the plus versions but from the Zahn 75 and Zahn 90 levels (ZahnBonus 5 module). Zahn 100+ differs from Zahn 100 only in professional cleaning: up to €110 per session and €200 a year instead of €90 and €180.
The premium is 0,44 € a month higher — 5,28 € a year. So you pay 5,28 € to get 20 € of extra limit. If you really do go for PZR twice a year, the plus version pays off. If you do not, you are paying for nothing.
Work out the premium for your age in a minute — the calculation commits you to nothing.
Zahnstaffel: the limits of the first four years
Like almost all dental tariffs, Ottonova caps payouts in the early years. It is the safeguard against people who sign up on the day they learn they need an implant.
| Period | Maximum total payout |
|---|---|
| 1st year | 1 250 € |
| years 1–2 combined | 2 500 € |
| years 1–3 combined | 3 750 € |
| years 1–4 combined | 5 000 € |
| From year 5 | no limit |
What the Staffel does not apply to. Prophylaxis (PZR, Fissurenversiegelung) is counted separately and is not capped by the Staffel. Treatment after an accident that happened while the contract was already running also falls outside the Staffel — and such payouts are not later counted against the limit for ordinary treatment.
At Ottonova the Versicherungsjahr is tied to the calendar year. The first insurance year ends on 31 December of the same calendar year; every later year equals a calendar year. In practice, a contract started in October is already in its second insurance year on 1 January. The limit opens up faster than it would if years were counted as rolling twelve-month periods.
Switching from another insurer. If in the six months before the contract you held a dental policy covering Zahnersatz with another insurer, the limits fall away after the second Versicherungsjahr rather than the fourth. Keep the proof of the old contract.
Health questions: there are only three
This is Ottonova’s strength. The application asks about exactly three things:
- How many of your teeth are missing and have not been replaced? Lückenschlüsse (gaps already closed by the neighbouring teeth) and wisdom teeth do not count.
- Do you have removable Zahnersatz?
- Has Parodontose been diagnosed, treated or recommended for treatment in the last 3 years?
Nothing else. No questions about caries, crowns, root canal treatment, implants already placed or dental visits in recent years.
Missing teeth decide everything:
| Missing teeth | What happens |
|---|---|
| none | standard premium per the table |
| one tooth | a 10 € surcharge on the monthly premium |
| two or more | no contract possible |
An important rule. Treatment that was begun or recommended before the contract was concluded is not paid for. This clause has no time limit: if a dentist wrote “implant indicated” in your file five years ago, that treatment stays outside the cover. This is why you sign the contract before seeing the dentist, not after.
What Ottonova Zahn 100 costs
| Age | Zahn 100 | Zahn 100+ |
|---|---|---|
| 0–17 years | 17,66 € | 18,10 € |
| 18–24 | 15,42 € | 15,86 € |
| 25–29 | 20,18 € | 20,62 € |
| 30–34 | 28,44 € | 28,88 € |
| 35–39 | 34,15 € | 34,59 € |
| 40–44 | 42,46 € | 42,90 € |
| 45–49 | 48,47 € | 48,91 € |
| 50–54 | 57,76 € | 58,20 € |
| 55–59 | 66,33 € | 66,77 € |
| 60 and older | 60,45 € | 60,89 € |
Premiums are shown without Risikozuschläge (risk surcharges). Two things you should understand in advance.
1. The premium follows your attained age, not your age when the contract was concluded. When you move into the next age band you automatically pay more: 34,15 € at 39, already 42,46 € at 40. That is not a premium increase but how the calculation is built: Ottonova sets aside no Alterungsrückstellungen (ageing reserves), so the premium rises in steps. After 60 it actually falls, to 60,45 €.
2. The premiums have not changed since December 2021. The table we show is dated 12/2021 — and it is the same table that is on ottonova.de today. We checked it in September 2026.
Your age, your premium. See the exact figure in a minute.
Kieferorthopädie: a lot for children, almost nothing for adults
Children up to 18. Groups KIG 1–5 are paid at 100 %, with a limit of 2 000 € over 10 Versicherungsjahre. KIG 1 and 2 are the decisive part. The Krankenkasse only starts paying from KIG 3, so parents carry mild misalignments entirely themselves. Ottonova Zahn 100 covers them.
Cover includes Invisalign, internal (lingual) braces, mini braces and plastic braces, colourless archwires, retainers, plus the imaging needed for treatment including digital volume tomography, and anaesthesia.
Adults from 18. Treatment is paid for in only two cases: if the Krankenkasse has accepted liability (severe jaw anomalies treated together with surgery) or if it is the consequence of an accident that occurred after the start of the contract. The ordinary wish to have your teeth straightened as an adult is not covered.
The limit of 2 000 € over 10 years is moderate. A full course of braces for a child costs 3 000–6 000 €; with KIG 3–5 the Kasse covers part of it.
What Ottonova does not pay for
- treatment begun or recommended before the contract was concluded — with no time limit;
- injuries from Kampfsport (martial arts);
- injuries in professional sport — if the sport provides at least 50 % of your income;
- treatment by a spouse, registered partner, parents or children (materials are reimbursed);
- deliberately self-inflicted damage;
- consequences of war in Germany, and abroad also consequences of war and terrorism if the German Foreign Office had issued a travel warning;
- general hospital services and single or double rooms during inpatient treatment;
- anything exceeding what is medically necessary or clearly out of proportion to the service provided.
And one separate clause worth remembering: if the Kasse paid nothing because a dentist without Kassenzulassung (no accreditation with the statutory scheme) was chosen, Ottonova reimburses 70 %, not 100 %.
Pros
- 100 % for Zahnbehandlung, Zahnersatz and implants — with no Eigenanteil (co-payment);
- only three health questions, and none of them is about caries, crowns or root canals;
- no Wartezeit: cover applies from day one;
- children’s Kieferorthopädie from KIG 1 — where the Kasse pays nothing at all;
- general anaesthesia, nitrous oxide, hypnosis and acupuncture up to 300 € a year;
- the Summenbegrenzung drops from four years to two when you switch from another dental policy;
- the premium has not changed since 2021;
- minimum contract term of 2 insurance years, with the insurance year tied to the calendar year; after that, cancellation at the end of the insurance year with 3 months’ notice
- children up to and including age 6 are accepted with no health check if a parent has been insured with Ottonova for at least 12 months;
- newborns — without a health check and without a waiting period if a parent has been insured for at least 3 months
- everything in the app: bills by photo, reimbursement, a concierge service for booking appointments.
Cons
- the premium rises when you move into the next age band: at 50–54 it is already 57,76 € a month;
- the GOZ factor is capped at 3,5 — anything the dentist bills above that you pay yourself;
- adult Kieferorthopädie only after an accident or with the Kasse involved;
- orthodontics limit of 2 000 € over 10 years;
- from the second missing tooth — the application is declined;
- professional cleaning is limited to €90 per session and €180 a year (€110 and €200 in the plus tariff)
- the tariff is strictly tied to GKV membership: if you move to private health insurance the dental cover ends automatically;
- premiums are debited only from a German account; card payment is not offered;
- at a dentist without Kassenzulassung the reimbursement drops from 100 % to 70 %.
Our recommendation
Ottonova Zahn 100 is a tariff for people whose teeth are in good shape and who want the matter fully covered rather than “partly reimbursed”. We recommend it:
- if no tooth is missing, or only one;
- if you do not want paper correspondence — all document exchange runs through the app;
- if there is a child in the family with a mild misalignment (KIG 1–2) that the Kasse will not pay for;
- if you are switching from another dental policy — the limit period is halved.
Ottonova Zahn 100 is not the right fit in two cases:
- if an adult needs orthodontic treatment without an accident — for adults Ottonova pays only after an accident or when the Kasse pays;
- if two or more teeth are missing — no contract is possible.
Between Zahn 100 and Zahn 100+, choose Plus if you really do go for cleanings: 5,28 € a year for 20 € of extra limit pays for itself with a single session.

FAQ: common questions about Ottonova Zahn 100
Does Ottonova Zahn 100 have a Wartezeit?
No. The Tarifbedingungen say explicitly: “Es bestehen keine Wartezeiten”. Cover applies from the start of the contract. Treatment begun or recommended before the contract was concluded is not paid for, however — that is a separate rule, and it does not expire.
How many health questions does Ottonova ask?
Three: how many teeth are missing and unreplaced (excluding wisdom teeth and Lückenschlüsse), whether you have removable Zahnersatz, and whether Parodontose was diagnosed or its treatment recommended in the last 3 years. Caries, crowns, root canals and implants are not asked about.
Can I take out cover if a tooth is missing?
One missing tooth — yes, with a 10 € surcharge on the monthly premium. With two or more, Ottonova does not offer a contract.
What is the Zahnstaffel (staggered payout cap) and how long does it last?
It is the payout cap in the early years: 1 250 € in the first Versicherungsjahr, 2 500 € over the first two, 3 750 € over three and 5 000 € over four. From the fifth year there is no cap. Prophylaxis and treatment after an accident do not count towards the cap. If you held another dental policy covering Zahnersatz in the 6 months before signing, the limits fall away after the second year.
Does Ottonova pay for adult braces?
Only in two cases: if the Krankenkasse has accepted liability (severe jaw anomalies treated together with surgery) or if it is the consequence of an accident that occurred after the contract began. Ordinary tooth straightening for an adult is not covered.
Is it true that the premium has not changed since 2021?
Yes. The Beitragstabelle is dated December 2021, and it is the same table that is on ottonova.de today — we checked it in September 2026. Inside the contract the premium still rises when you move into the next age band — that is not a premium increase but how the calculation is built.
What happens to the dental policy if I switch to PKV (private health insurance)?
The contract ends. The tariff is open only to members of a German Krankenkasse. As soon as GKV membership ends, so does the Ottonova dental policy. In PKV, dental cover is usually already part of the main tariff.
Zahn 100 or Zahn 100+ — which to choose?
The only difference is the limit for professional cleaning: €90 per session and €180 a year versus €110 and €200. The plus version costs €0.44 more a month, i.e. €5.28 a year. If you have a cleaning at least once a year, take the plus version — it pays for itself. If not, there is no point.
Is Zahn 100 the right fit? Applying takes about 10 minutes and is entirely online.
